Needs Assessment – Discovery Meeting Our services begin with a full understanding of both the elder person and their family. When possible, we visit each prospective client in the home, hospital, assisted living facility, or any other setting.
   
Craft Personalized Care Plan
We craft a personalized, at home Care Plan that includes a range of non-medical services specially designed to accommodate the needs of the entire family.
   
Match Clients Needs with Appropriate Caregiver
We ensure the best match is made between the elder's needs and caregivers skills, experience and personality.
   
Monitor In-Home Care
We provide quality assurance under the direction of a Registered Nurse (RN) – guides and assists CAREgivers along with periodic visits to ensure compliance with our care management standards.
   
Communication
We maintain ongoing communication with the client and their family member (including healthcare professionals when requested). We provide a daily care log of all activities and monthly reports to family members.
